A ladder, not a program.
Every rung pays in money. Two rungs bill Medi-Cal. One is credentialed specifically on having lived through it. Participation is always voluntary — help is never conditioned on work.
| Tier | Role | What they do | Credential |
|---|---|---|---|
| T1 | Community Outreach Worker | Canvassing, trust-building, finding neighbors, renewal outreach | None. We hire and train. |
| T2 | Community Health Worker | Navigation, education, renewals, benefits and patient-assistance applications | Certificate or 2,000-hour pathway — billable |
| T2+ | Certified Peer Support Specialist | Recovery-oriented peer support | State certification built on lived experience — billable |
| T3 | Navigator Lead | Caseload management, escalation triage, documentation quality | Experience + internal promotion |
| T4 | Licensed Social Worker | Behavioral health assessment, complex psychosocial cases | LCSW / ASW |
| T5 | Nurse Navigator | Triage, medication reconciliation, care plans | RN |
| T6 | Medical Director | Protocols, standing orders, clinical governance | MD / DO / NP |
The scope line. T1–T3 collect structured, non-diagnostic information only. They never diagnose, advise, or triage. Red flags escalate automatically to T5–T6. This protects the neighbor, protects the navigator, and makes the billing defensible.
Jobs we intend to open
These are not open positions. We are in formation and not yet funded to hire. Register interest and you get the first call.
Community Outreach Worker
Hourly, W-2, health coverage. Local residents strongly preferred, Spanish essential. No degree, no experience. We train you and your hours count toward certification.
Community Health Worker
Hourly, W-2, health coverage. Certificate or 2,000 hours of prior community work. The role the model grows people into.
Peer Support Specialist
For people with lived experience of recovery. State-certified and billable — your experience is the qualification.
Navigator Lead
Bilingual. Community health or case management background. Promotion path from T1–T2.
Nurse Navigator (RN)
Active California RN license. The clinical backbone and escalation point for every navigator in the field. We need this role early.
Volunteer
Renewal outreach, translation, paperwork. The one thing that does not wait on funding — and never a condition of receiving help.
